1.  Choose your favorite photo.

  Tips:  Select a good quality photo, blurry images do not reproduce well.  Slose ups and minimum objects 

produce the best results.  Your account number will be printed across the lower third of the card and tcover a protion or your 

photo.

2.  Trim photo to a square of 3" wide by 3" high.  IMPORTANT:  If the photo does not fit these exact dimensions, the photo may 

have to be cropped, resized, or rejected.

Please keep a copy of your photo.  We are unable to return your submitted photo.
